Please note:<ul><li>If you change your vehicle you must complete a Car Parking change form and return it to car.parking@cht.nhs.uk</li> <li>If you own more than one vehicle, please note only one Staff Parking Permit will be issued and must be transferred between vehicles.</li>
<strong><u>In accordance with the Trust’s Parking Policy, permits will be allocated on the basis of greatest need using the criteria set out on the next page.</u></strong> If you are successful, a permit and swipecard will be sent to General office to be collected and signed for. Unsuccessful applicants will receive a letter advising them of the Appeals Procedure. <span id="red"><u >Please do not ring the Travel Department as they cannot deal with queries over the telephone.</u></span> If you wish to cancel your permit at any time, please send your permit and swipecard back to the General Office F.A.O. the Travel Department no later than the 4th of the month so that payments can be stopped. <strong><u>This includes long term sick leave and maternity leave. </u></strong> Alternative ways to pay will need to be sought if you do not work for CHFT i.e. by Invoice or through SWYMHT or Locala Payroll.
