In line with guidance issued by the Disclosure and Barring Service (DBS) which is an organisation whose purpose is to conduct checks and provide details of criminal records and other information, your post has been identified as one that requires a check to be carried out by the Trust. I should be grateful if you would call into the Workforce and Organisational Development Directorate and, bring with you the following documentation. At the same time, a DBS form will be given to you for completion:
- Passport and Photo Driving Licence
- Birth Certificate
- P45 or P60
- Two proofs of address e.g. recent utility bill, credit card or bank statement
- Confirmation of Qualifications (e.g. Certificates)
DBS checks are now only issued to the applicant, not the prospective employer. When you receive your certificate it is important that you contact the Trust on the above number. You will need to present the original certificate. Failure to do so may delay your appointment or cause the job offer to be withdrawn.
Please be aware that you will be charged for this check and monies will be deducted from your salary on commencement in post. You will be required to participate in the DBS Update Service at a cost of £13.00 which you will also be required to pay. For further information on the DBS Update service, please click on the link. If you have registered with the DBS Update Service please produce your certificate and complete the DBS update section in the reply section of this letter.
