This form is to be used for colleagues wish to have a bank contract setting up and issued. On receipt of the request recruitment will review the employee record and ensure employment checks are valid and in date. Adding a bank assignment creates an employment change which may require colleagues to complete additional check prior to bank work being paid. This included DBS requirements (correct level on the DBS update service) and ID documentation.
