Certificate of Sponsorship (CoS) Request Form

This form is to be used for colleagues and new colleagues who require a renewal or a new Certificate of Sponsorship from to work at CHFT.   

On receipt of the request, Recruitment will review the form and request the CoS.

By saying yes on the CoS, you agree to certify maintenance for both the worker and (if relevant) any dependants of the worker. If you certify maintenance for dependants, you confirm that you will maintain and accommodate them for the first month of any permission that is granted to them.
